Mission:  LGBT Life Center is a trusted leader that empowers the LGBTQ communities and all people affected by HIV through improving health and wellness, strengthening families and communities, and providing transformative education and advocacy. 

Essential Job Functions:

  • Works closely with clients living with HIV/AIDS through a strength-based approach to support the multiple health, psychosocial and other service needs.
  • Provides medical case management services, including treatment adherence, to offer a range of client-centered services that link clients with healthcare, psychosocial and other social services needed.
  • Coordinates effective and affirming relationships with various care teams to ensure treatment adherence and retention in care for all clients.
  • Completes and develops Individualized Service Plan (ISP) and coordinates with internal/external services to ensure medical, housing stability, behavioral health support, food, transportation, enrollment in LIFE Pharmacy program, and access to ongoing mainstream resources.
  • Provides excellent engagement with clients by meeting with them on a regular, ongoing basis which includes home visits and shelter visits as needed.
  • Participates in internal program team meetings/trainings to ensure coordinated efforts for client stability.
  • Carry case load of MCM clients of 50-55 clients

Responsibilities:

  • Maintains individual files and excellent documentation, including all written documentation related to essential functions such as eligibility, case notes, services plan, assessment, enrollment in healthcare, access to medication (LIFE Pharmacy or other partner pharmacies in the region.
  • Supports program expectations and performance milestones in accordance with contracts guidelines and expectations
  • Provides accurate, complete, and timely data and reports to the Program Manager as needed.
  • Records data for program outcomes and ensures client files are complete, accurate and frequently updated.
  • Participates in various agency-wide meetings and events to support client and community engagement.
  • Assist with Ryan White Quality Management and Improvement projects.
  • Other duties as assigned.

Qualifications:

Education:  Bachelor’s degree in Human Services or a related field is preferred.  We might substitute a bachelor’s degree for extensive practical experience.

Experience: Minimum of 2 years’ experience directly related to duties/responsibilities specified.  Experience providing HIV/AIDS services, case management and/or coordination navigation preferred.

Job Knowledge and Skills:  This position requires knowledge and sensitivity to HIV/AIDS and other target populations (LGBT, homelessness, mental illness, substance abuse) along with the ability to work in a clinical and community environment.  Must possess the ability to identify and communicate effectively with the members of the target population. Ability to work with multicultural and diverse population required.  Effective oral and written communication skills required.  Excellent organizational, decision making, problem solving, and time management skills are required to handle multiple activities related to program implementation.  High energy level and self-motivated. Knowledge of “best practices “in status neutral approach and how they apply to the programs we provide. Computer knowledge should include Microsoft applications (Word, Excel, Outlook).

Physical Requirements:

Must have reliable transportation and acceptable driving record is required.  Transporting of participants is required.

This work requires the following physical activities:  walking, sitting, hearing/seeing, talking in person or on the phone.  Occasional walking, bending, standing, and driving are needed.  Work is performed in an office setting, and in home environments.  Work is also performed in social and community settings, as necessary.  Minimal to moderate lifting is required at times.  Extended work schedule may apply to accommodate client’s needs.

Salary: Non-Exempt, full-time hourly position with pay rate at $45-48K annually based on experience and qualifications. Comprehensive benefit package including PTO, 403B, Medical and dental coverage.

Preferred location:   Applicants must live within 40 miles of LGBT Life Center offices in Norfolk, VA.  This is an on-site and field position.
