Description

 Working within a public health program grounded in the connection between stable housing and health outcomes, the Housing Case Manager provides high-quality, comprehensive, and intensive case management services to people living with HIV, and youth who are exiting homelessness while they are residing in a scattered site or project-based supportive housing program. Build trusting relationships with clients, which will help them maintain housing, improve health outcomes, and reach personalized goals. The Housing Case Manager supports and empowers clients by utilizing Housing First and Harm Reduction frameworks, Motivational Interviewing, and Trauma Informed Care. 

Responsibilities:

  1. Provide acuity-based case management and housing stabilization services for clients residing in a scattered-site or project-based supportive housing program from the Boston and Cambridge locations. Develop client-centered service plans based on client needs after conducting comprehensive assessments and reassessments. Assist clients with accessing health care, income and benefits, housing search, and other resources. 
  2. Provide clients with easily accessible, low-threshold, and trauma-informed services that operate within the harm reduction framework. Provide assistance to overcome barriers to service delivery and health care access by serving clients on a walk-in, scheduled, and as-needed basis both in person and virtually.
  3. Identify, coordinate and advocate for appropriate support services and resources that will assist clients in optimizing health outcomes and moving towards self-sufficiency including medical care, housing, financial, mental health, substance abuse, sexual health, legal, vocational and educational services. With the support of the organization, develop and maintain knowledge of resource networks for youth, and people living with HIV in the Greater Boston Area.
  4. Maintain communication with internal and external providers to improve access to health care, accompaniment to medical appointments, conduct annual home visits and meet in the community as needed. Form part of a collaborative care team and coordination of services with relevant providers and family members, as needed and with client consent. 
  5. Work closely with clients on all phases of the subsidized housing process, including completing and following up on housing applications, researching available apartments, and accompanying clients to all related housing appointments such as interviews, apartment viewings, and lease signing. 
  6. Assist clients with all aspects of eviction prevention. Accompany clients to court and appeal hearings. Assist with accessing rental assistance programs, utility assistance, and other stabilization services related to housing stability. 
  7. Accurately record and maintain up-to-date client records including assessments, individual service plans, progress notes, clinical indicators, and all other required paperwork in Fenway Health’s electronic medical record database, Epic. 
  8. Attend and participate in scheduled program, department, agency, and collaborator meetings, weekly supervision, and trainings.
  9. Perform other duties as required.
Requirements
  • High school diploma or GED
  • Bachelors, or associates degree or 2+ years of equivalent experience
  • Excellent written and oral communication skills.
  • Working knowledge HIV/AIDS community trends, harm reduction, motivational interviewing and at-risk populations.
  • Ability to work harmoniously and effectively with colleagues, patients, clients and vendors across the spectrum of diversity, including but not limited to race, ethnicity, color, gender identity, sexual orientation, age, socio-economic status, national origin and immigrant status, religious or spiritual identity, disability (physical, mental, emotional and developmental), veteran status, and/or limited English proficiency.

Preferred Qualifications:

  • Experience with the provision of HIV services.
  • Experience with affordable housing.
  • Experience advocating for clients with government assistance agencies, housing authorities, private landlords or realtors.
  • Valid Massachusetts driver’s license

$24.62-30.77 per hour ($48,000-60,000/year)
